Set in a dystopian, near-future society, the film follows Bob Arctor, an undercover narcotics cop who becomes deeply embedded in the counterculture of "Substance D"—a highly addictive drug that causes severe brain splitting and hallucinations. To protect his identity, Arctor wears a "scramble suit" that constantly alters his appearance, preventing even his superiors from knowing who he is. As Arctor sinks deeper into his undercover role and develops a dependency on the drug, the line between his real identity and his undercover persona blurs, leading to a surreal descent into paranoia, surveillance culture, and corporate conspiracy.

The film is visually distinct for its use of rotoscoping, a technique where digital animators traced over live-action footage frame by frame, perfectly capturing the disorienting, hallucinatory state of the characters.
